In 2005, at the age of 25, she was diagnosed with ovarian cancer and informed that she may not be able to conceive children. She had to have multiple surgeries to remove tumors from her ovaries, which took place over a period of two years. After her last surgery, she and her husband Taran Killam discovered they were going to have their first child within a year.

Smulders and Killam welcomed their second daughter, Janita, in January 2015. It was a unique experience for Smulders as she was pregnant with Janita while filming Unexpected. She even discussed the challenges and joys of being pregnant while working on set during the promotion of the movie at the Sundance Film Festival in 2015.

Janita has had the unique experience of growing up on set with her mom. Smulders often shares glimpses into her younger daughter's life, showing their favorite activities like watching documentaries and The Great British Baking Show, listening to musicals like Hamilton and Annie, hiking, and playing with vegan modeling clay.
